Band 6 CPN - Mental Health Outreach Team for Rough Sleepers

About the Role

A Vacancy at Birmingham and Solihull Mental Health NHS Foundation Trust.

To work in the mental health outreach team for homeless rough sleepers located in the city centre but meeting the needs

of homeless people city wide.

To be responsible for the engagement with and assessment of mental health needs of service users who are identified as

rough sleeping in the city via the Rough Sleeping Task Group

To refer service users to BSMHFT secondary care services if required and liaise with those services to facilitate care
