About the Role

To work as a skilled and autonomous practitioner within the All Age Continuing Care (CHC) service. The post holder will

be responsible for the assessment, commissioning, and review of care packages for individuals with complex, ongoing

healthcare needs, ensuring the effective application of the National Framework for Continuing Healthcare and adherence

to all relevant clinical guidelines and legislation. 2. Main Duties and Responsibilities Clinical Practice & Assessment:

To undertake comprehensive, face-to-face assessments for individuals who may have a primary health need, utilising the

Decision Support Tool (DST) as part of a multi-disciplinary team. To apply a robust understanding of the National
