About the Role
An exciting opportunity has arisen for a suitably qualified and experienced senior mental health practitioner to work
closely with the Team Manager and other senior clinicians within H&F Central MINT services in Hammersmith & Fulham. This
post is open to nurses, social workers and occupational therapists and is a management post within the team.
Remodelled community teams have been established over the past year following a successful bid for additional resources.
In line with the population health model the team now deliver mental health interventions in partnership with primary
care colleagues as well as continuing to provide care for those with more complex needs.
