Locality hubs for a Band 7 Clinical Practitioner for Planned care to support the Locality Integrated Neighbourhood team.

We are seeking an experienced Clinician with a strong background in the management of frailty and long-term conditions.

The successful candidate will play a key role in leading high-quality, person-centred care, working collaboratively

across multidisciplinary teams, and contributing to service development and improvement.
