About the Role
Job Introduction
We are seeking an experienced and motivated Registered Nurse, Occupational Therapist or Physiotherapist to lead our
Community Transfer of Care Hub (CTCH).
This is a pivotal Band 7 leadership role for a discharge specialist with expert knowledge in complex discharge planning,
patient flow and system-wide working. You will lead and manage a team supporting patients with complex needs to
transition safely from hospital into community placements, packages of care or community beds.
