About the Role
An exciting opportunity has arisen for a suitably qualified and experienced Senior Pathway Coordinator to work closely
with the Service Manager, Team Manager and other Senior Clinicians within H&F Central, North & South MINT services in
Hammersmith & Fulham. This post is open to all Health Care Professionals, Nurses, Social Workers, Occupational
Therapists and is a management post within the team.
Remodelled community teams have been established over the past few years following a successful bid for additional
resources. In line with the population health model the team now deliver mental health interventions in partnership with
