About the Role
We are looking for an enthusiastic professional who can help the Community Independence Service (CIS) make Hammersmith &
Fulham (H&F) a great place to grow old.
The Community Independence Service is co-located with London Borough of Hammersmith and Fulham (LBHF) Adult Social Care
on King St, Hammersmith as well as within Charing Cross Hospital.
Our service works closely with our acute partners, mainly Charing Cross but also other acute Imperial College Healthcare
NHS Trust sites and Chelsea & Westminster Hospital NHS Foundation Trust.
