About the Role

We are looking for for an experienced, well organised, Registered Healthcare professional to cover the Maternity leave

of our current Discharge Hub Coordinator.

The post holder will be an experienced clinician or Social Work practitioner working within the mental health, learning

disability and autism team (MHLDA) and will sit within the Birmingham and Solihull MH&LDA Provider Collaborative.

The post holder will be expected to work alongside senior clinical leads and colleagues to provide oversight and

co-ordination of the inpatient cohort including admission avoidance strategies and facilitating the discharging of
