Provide and maintain a high standard of skilled nursing care for patients in their own homes, health centres or practice
surgeries, in compliance with protocols and guidelines. Assess and re-assess patients with non-complex and complex
conditions, reporting back to the District Nurse on any interventions undertaken. Respond effectively to the local
practice population health needs through working within the primary health care and social care team. Promote health in
all age groups by advising and supporting clients and families. Communicates observations of physical, mental and social
