About the Role

The post holder will work autonomously and in collaboration with other health care professionals, statutory and

voluntary agencies, and the planned care locality teams to deliver excellent standards of patient care for patients

requiring support in unplanned care.

• To Deliver high quality, skilled care to patients at home.

• Work shift patterns including weekends and bank holidays. The service operates through pre-rostered shift patterns

spanning 8AM – 10PM 7 days a week.
