About the Role

1.Interdisciplinary working with other health professionals for holistic assessments and treatment plans including

assessment, goal setting, planning and developing therapy treatment programmes; monitor and review, as part of the

Physiotherapy component of the D2A Pathway 1 Service. Responsibilities of this post holder will include the following:

1.To write and review treatment plans to enable patients to maintain maximum independence. 2. To undertake treatment

plans identified by physiotherapists. 3.Act at all times in a manner which respects privacy, dignity and

confidentiality. 4. To maintain patients notes in accordance with Trust policy. 5. To manage own caseload, time
