About the Role

Main Duties and Responsibilities: Clinical responsibilities: IUC Hub/Clinical Assessment Service: Consistently monitor

incoming calls, responding promptly based on clinical need Provide telephone assessment for patients presenting via 111,

closing cases over the phone where appropriate Provide clinical advice for Health Care Professionals and co-ordinate

care for patients depending on clinical need Arrange to review patients face to face at the appropriate UTC/SDUC clinic

Arrange home visits when clinically needed Provide clinical advice where appropriate to both internal and external staff

as required eg triage nurse, GP colleagues, district nurses, ACPs, Paramedics etc Through prior agreement and where
