About the Role

Deliver personal care and assistance with daily living to recently discharged patients Support medication administration

and accurate documentation Observe, record, and escalate changes in condition following care plans Assist with mobility,

nutrition, and other essential care needs Communicate effectively with service users, families, NHS staff, and social

care teams Maintain accurate records using our electronic care record system Adhere to infection control, safeguarding,

and health & safety procedures Essential Requirements: Right to work and live in the UK (UK Citizen or Indefinite Leave

to Remain) Basic Life Support & Moving and Handling (practical) Able to evidence a clean DBS Provide satisfactory
