About the Role
Main Duties Work under the direct supervision of the RN and in line with the patients care plan, providing care to meet
fundamental health care needs. This includes assisting patients with toileting, personal care and supporting patients to
eat and drink as independently as possible whilst maintaining privacy and dignity at all times. Following induction and
assessment of competence, the post holder undertakes delegated visits independently in line with written care plans,
with clear escalation routes to the Registered Nurse or Hospice at Home Lead. Medication support is provided following
completion of medicines support training and competency assessment, in line with hospice medicines policy. This includes
