About the Role
Main Duties Clinical Care Holistic assessments are undertaken in patients homes, with delivery of individualised nursing
care in line with assessed needs and agreed care plans. Care plans and clinical documentation are reviewed, updated, and
maintained in accordance with organisational requirements. Safe, effective symptom management and end of life care are
provided, including recognition and escalation of deterioration and symptom change. Integrated Working and Care
Coordination Partnership working is maintained with the Integrated Hub, Hospice at Home Lead, Care Navigators, and wider
system partners to support coordinated care. Autonomous practice is exercised within scope of professional competence,
