About the Role
The post holder will: Work in partnership with the local Health Visiting service to ensure seamless care as the child
progresses from the early years into school. Have the ability to recognise health needs for individual clients and
situations in relation to the health of children, young people, individuals and families, sometimes in complex
situations, identifying situations where referral for support from Public Health Specialist is necessary. Undertake
health assessments in schools, or homes where appropriate. Develop factual reports, attend and participate in relevant
case conferences and inter-agency meetings for CYP on their caseload, and undertake health needs assessments and care
