About the Role

Central London Community Healthcare NHS Trust (CLCH) delivers the Stroke and General community in-patient rehabilitation

service for Barnet as well as offering some step-down and non-weight-bearing beds across the wards.

The units provide a comprehensive community-based step-down facility that supports patients’ rehabilitation and recovery

in a community setting, with an active therapy programme.

Ongoing assessments for those patients who are medically optimised but who require further assessments (typically

Continuing Healthcare (CHC) assessments) outside of the acute setting in relation to their health and social needs and
