CAMHS/AMHS Transition Practitioner | South London and Maudsley NHS Foundation Trust
About the Role
This is an exciting opportunity to work alongside the CAMHS and Adult mental health services to support the transition
of young people from CAMHS to Adult services. The main responsibility of the post holder is to improve the relationship
between the two partners as well as the wider organisation and external partners to best support our young people in
Lewisham. The post holder will be supported by the Service manager within an adult community centre and supervision will
be provided regularly via the existing CAMHS/AMHS forum and other reflective platforms. The caseload will be allocated
to the CAMHS transition worker following referral from CAMHS and service users will handed over after the stabilisation
