Cancer Care Coordinator - 15 hours

The post holder will support the implementation of the Personalised Care Agenda for people affected by or living with

Cancer, including:

• Working closely with the cancer nurse specialists by:• assisting providing Holistic Needs Assessments and producing

associated care plans,

• supporting the implementation and administration of Treatment Summaries,
