About the Role

Job purpose The post holder will provide expert, specialist cardiovascular nursing care within general practice,

delivering high-quality, evidence-based assessment, treatment, and ongoing management for patients with cardiovascular

disease. The role focuses on secondary prevention, long-term condition management, and medicines optimisation,

supporting improved clinical outcomes and continuity of care for patients. This is not a development role. The post

holder will be an experienced cardiac nurse specialist, working with a high level of professional autonomy and clinical

responsibility, and contributing specialist knowledge to the wider practice team. Main duties and responsibilities
