About the Role
Join Inclusion Healthcare as a Care Coordinator, where you'll play a vital role in providing compassionate care to the homeless population in Leicester. You'll coordinate patient care, facilitate communication among healthcare professionals, and empower individuals to manage their health effectively.
Responsibilities
- Coordinate care for homeless patients across health services.
- Liaise with agencies to enhance patient engagement.
- Provide trauma-informed support and navigate complex systems.
- Maintain accurate patient records in SystmOne.
- Participate in multidisciplinary team meetings for integrated care.
Requirements
- Experience in care coordination or similar roles.
- Strong communication and interpersonal skills.
- Ability to work with vulnerable populations.
- Proficiency in IT systems, including SystmOne.
- Commitment to ongoing professional development.

Compensation
Salary is £13,250 per year for 18.75 hours per week, equivalent to £26,529 FTE.
Location & Schedule
Based at Charles Berry House, Leicester, with part-time hours of 18.75 per week.
