About the Role

Join Inclusion Healthcare as a Care Coordinator, where you'll play a vital role in providing compassionate care to the homeless population in Leicester. You'll coordinate patient care, facilitate communication among healthcare professionals, and empower individuals to manage their health effectively.

Responsibilities

• Coordinate care for homeless patients across health services.
• Liaise with agencies to enhance patient engagement.
• Provide trauma-informed support and navigate complex systems.
• Maintain accurate patient records in SystmOne.
• Participate in multidisciplinary team meetings for integrated care.

Requirements

• Experience in care coordination or similar roles.
• Strong communication and interpersonal skills.
• Ability to work with vulnerable populations.
• Proficiency in IT systems, including SystmOne.
• Commitment to ongoing professional development.

Compensation

Salary is £13,250 per year for 18.75 hours per week, equivalent to £26,529 FTE.

Location & Schedule

Based at Charles Berry House, Leicester, with part-time hours of 18.75 per week.
