Care Co-ordinator

About the Role

CARE CO-ORDINATOR RESPONSIBLE TO: Practice Manager OVERVIEW This is a role established to work with vulnerable patients

from our practice location, both remotely and in person. You will be working closely with our Health Co-ordinator team

and clinicians to support the wellbeing and care needs of our patient population. You will have access to general advice

and support and will also have clinical team oversight or supervision from a Partner where necessary. MAIN PURPOSES To

provide wide-ranging, proactive and efficient administrative support to Cheriton Bishop & Teign Valley Practice To be

responsible for the administration, co-ordination and monitoring of dedicated or targeted clinical campaigns To work
