About the Role

The Community Forensic Outreach Service (FOS) has a vacancy for a Forensic Community Psychiatric Nurse and Care

Co-ordinator. The core of the job is the management of a caseload of patients with forensic histories, acting as their

care co-ordinator and as social supervisor as appropriate. You will alongside an MDT including Doctors, Psychologists,

Recovery Workers, OTs and Social Workers.

This post holder will be engaged in effective risk assessment and management, as well as leading on engagement and

interventions to support service user recovery and achievement of service user defined goals.
