About the Role

Job Title: Care Coordinator (Care Operations) Reporting to: Head of Care (Nursing) Location: Horwich, Bolton with travel

across the North West Hours: 35 hours per week (Monday Friday, 9:00am 5:00pm) On-Call: 1 weekend per month on a rolling

rota Role Purpose The Care Coordinator is responsible for supporting the operational delivery of care services, ensuring

all care packages are effectively staffed, coordinated and delivered to a high standard. Working closely with the Head

of Care, the Care Coordinator ensures day-to-day operational readiness, staff coordination, training oversight and

quality compliance across all active care packages. The role works collaboratively with Team Leaders and Senior Carers
