We are looking for an enthusiastic individual to join the team and undertake a newly created Care Coordinator role -

completing safe and efficient allocation of urgent patient visits.

To undertake the safe and efficient co-ordination of urgent patient appointments for Urgent Community Response North. To

audit and monitor electronic information to ensure accuracy and quality. To maintain good customer care when

communicating with the public and health/social care professionals.
