About the Role
Choices Healthcare are looking for a reliable and motivated individual to join our company as a Care Manager with scope
to progress to a Registered Care Manager (RCM) within 6-12months.
As a Care Manager you will provide a tailored, responsive and high quality service to individuals with various care
needs. You will effectively manage a team of staff to deliver person centred and specialist services that meet our
organisational standards and expectations.
You will work with families, social workers and other professionals to monitor and review each individual clients'
