Care Navigator
About the Role
Main Duties Care Navigation and Patient Support Act as a first point of contact for patients, carers and professionals
contacting the End-of-Life Care Together Hub. Provide clear information about services and support options and help
people navigate these. Support patients and families to understand and follow their Advance Care Plan and preferred
place of care. Offer ongoing practical and emotional support, recognising changing needs and escalating to clinical
staff when required. Work in partnership with primary and secondary care colleagues, members of the End-of-Life Care
Together Alliance, the Beaumond House team and other support roles. Referral Management and MDT Coordination Receive and
