About the Role
As a Care Navigator, you will support clinicians by ensuring a seamless patient experience. Your role involves welcoming patients, managing inquiries, coordinating appointments, and facilitating communication between patients and healthcare providers. This position is vital in maintaining high-quality service within the practice.
Responsibilities
- Greet and direct patients upon arrival
- Manage telephone inquiries and requests
- Coordinate home visit requests for doctors
- Book patient transport for hospital appointments
- Register new patients in the clinical system
- Initiate computerized prescriptions as needed
- Assist in data entry and patient follow-ups
Requirements
- Strong communication and interpersonal skills
- Experience in a healthcare setting preferred
- Proficient in using clinical software
- Ability to multitask and prioritize effectively
