Care Practitioner

About the Role

You will work as part of a team supporting adults with autism and complex behaviours in their own homes in the

community. Duties include all aspects of personal care, supporting with all daily living tasks including medication,

housework and cooking. Staff must have the physical ability to undertake Physical Intervention training and Escape

techniques training. You will act as keyworker, including monitoring and ensuring the service user is being offered the

opportunities, experience, care and support identified in their support plan. The role will include keeping accurate

records of outcomes focused support planning and review for the people whom you support, ensuring changes in service
