Case Manager - Individual Commissioning Learning Difficulties
About the Role
Job Summary The postholder is accountable to the Associate Director Individual Commissioning and responsible tothe Case
Management Team Leader. The postholder will be expected to work across the breadth of specialties according to their
competencylevels. For example, all of the team will be expected to cover Personal Health Budgets (PHB) orappeals, but
only those competent to cover childrens or Learning Disabilities (LD) would be expectedto do so. The postholder will
take an active role and work as an expert clinician in facilitating and/or providingprofessional support, commissioning
and case management of patients eligible for NHS ContinuingCare, NHS Funded Nursing Care, CYP Continuing Care, those in
