About the Role
A band 6 case manager opportunity has arisen within the North Hull Community nursing team, The successful candidate will
be responsible for supporting the integrated community nursing team to deliver holistic high quality nursing care to a
dedicated client group in their own homes and residential settings.
You will be joining a dynamic and proactive nursing team working in partnership with associated health and social care
providers. This is an ideal opportunity for those who enjoy developing new ways of working and working under their own
