About the Role

This is a great opportunity to build your clinical and leadership skills while supporting people with complex health

needs in the community.

You’ll work as part of an experienced community nursing team to provide personalised care for people living with

long‑term conditions. Your work will help people stay well at home, avoid unnecessary hospital admissions, and feel

confident in managing their health. Working closely with patients, carers and partners across health and social care,

you’ll play an important role in shaping safe, coordinated, and responsive care.
