About the Role

The Urgent Care Service prevents avoidable hospital admissions

People unwell at home may require an urgent response or increased level of support to help them avoid a hospital visit.

GP’s, and other healthcare professionals may refer to our service if they become concerned about your health and

welfare.

We aim to respond to referrals within two hours and will arrange to visit to assess, diagnose, treat and implement

carestrategies that will enable patients to remain safe and well in the comfort of their own home. In many cases, we are
