About the Role
Home), Long Term Conditions, Training, Home Respite and the Nurse led Continence Service. The CATCH Team provide a
service from 8am-9pm 7 days a week. The post holder will work as part of a team assessing acutely unwell children in
their own homes/community, taking referrals from GPs, EMAS and the acute hospital. With the aim to reduce hospital
admissions and manage the child safely within the community setting. The post holder will work as part of a team whilst
working autonomously in practice in both GP hubs and community settings as first contact assessing, diagnosing &
managing and prescribing for children under 6 years of age presenting with acute illness, along with providing an
