About the Role

We are seeking an experienced and compassionate Specialist Palliative Care Social Worker (Band 7) to join the Adult

Community Services Specialist Palliative Care Team for the next 6 months.

This rewarding and specialist role focuses on providing expert social work support to people living with life-limiting

illness, and their families and carers, during times of significant change, uncertainty, and loss.

Working as a core member of the multidisciplinary Specialist Palliative Care Team, you will collaborate closely with

Clinical Nurse Specialists, Consultants and other professionals to deliver holistic, person-centred support across a
