About the Role
Job Title:CHC Nurse AssessorBand:6Department:Continuing Healthcare (CHC) TeamLocation:Hybrid working (typically split
between home and one of our main offices across BSW, e.g., Chippenham, Bath, or Swindon). Travel across the BSW region
will be required. Main Responsibilities: Clinical Assessment & Decision Making: To act as the nominated lead,
coordinating and undertaking comprehensive assessments for NHS Continuing Healthcare in line with the National
Framework. To facilitate Multi-Disciplinary Team (MDT) meetings, ensuring the effective completion of the Decision
Support Tool (DST). To apply professional and clinical knowledge to assess the nature, complexity, intensity and
