CHC Nurse Assessor

About the Role

To be an integral part of East Berkshires Continuing Healthcare and Funded Nursing Care Team. The postholder will

implement the National Framework for Continuing Healthcare and NHS Funded Nursing Care. To complete high quality

holistic assessments, reviews and re-assessments in a variety of settings i.e., nursing home and community to enable

application of the NHS National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. Face to face

reviews/assessments will be required. Work as part of a multi-disciplinary team, acting as a resource to provide nursing

advice in relation to appropriate placement and management of clients. Provide accurate advice and information to all
