Children Community Nurse

About the Role

This role aims to reduce hospital admissions, support safe discharges, and enable children and young people to live as

fully and independently as possible within their communities, with dignity, choice, and inclusion at the centre of care.

To work in collaboration with partner agencies both voluntary and statutory to achieve good health outcomes for children

and their families. To ensure the delivery of the quality strategy within the designated area incorporating

establishment of systems and processes.
