The role requires case managing, caring for children with a chronic or acute nursing need outside of the hospital

environment, working within a nurse led team. It is an opportunity to share your ideas and improve patient outcomes.

This is a maternity cover, Full time 37.5 hours per week (some weekend and bank holiday working required) - 12 Months
