About the Role

To undertake comprehensive assessments of need for children with complex health needs, utilising specialist health

knowledge and experience. Always maintain a person centred and multi-agency approach to needs assessment and care

package development. To work in partnership with all relevant stakeholders in the provision of continuing health care

packages to enable the child or young person to attain their optimum life experience and outcomes. To facilitate

difficult and challenging conversations and communicate highly sensitive and complex information using high level

communication skills to a range of audiences including medical/non-medical professionals, parents/carers, children/young
