Children's Continuing Care Nurse
About the Role
Main Duties and Responsibilities To assess, implement, deliver and evaluate programmes of nursing care to children and
young people adopting a flexible holistic approach to meeting individual needs. Supporting children / young people and
their families in meeting identified health needs, either in the family home, or other community settings as
appropriate, in collaboration with other multi agency providers across BLMK. Planning need to be able to plan and
deliver evidence based skilled nursing care to children on the CCCT caseload. Be able to write comprehensive care plans.
Patient contact work in partnership with children, families and carers to provide the optimum individualised care in the
