Children's Continuing Care Nurse
About the Role
Provide direct care to children with complex health needs in the home setting. To train and develop Care staff through
competencies to ensure they are sufficiently skilled to care for specific children on the complex care caseload. To
assist the senior practitioner in the supervision, teaching and supporting of a team of associate carers that provide
health care support for children dependent on technology. Supporting the Senior Practitioner in the review of care plans
and outcomes, contributing to the regular reports for managers and commissioners. Involve parents and young people in
service review. To attend staff and other relevant meetings/team days. To demonstrate a commitment to implement elements
