About the Role
An exciting opportunity has arisen to join a well-established Children’s transition team at Colchester. The band 7
position is a lead role with line management responsibilities. The role ensures that young patients and their families
are well-supported during this critical phase, helping manage their medical needs, build confidence, and navigate the
often challenging shift between healthcare systems.
The Nurse Specialist works closely with multidisciplinary teams to deliver individualised care plans, empowering young
people to take control of their health and promoting better long term outcomes.
