CHSW Liaison Nurse - CFT (Secondment/Temporary 18 months)
About the Role
Working with the Children and Young Peoples Community Palliative Advance Nurse Practitioner and community childrens
nursing team, alongside the lead palliative care nurse and the medical and nursing leads at Childrens Hospice South West
(CHSW) the post holder has a key role in the continued delivery and development of palliative care services within
Cornwall and CHSW. The primary purpose is to promote a close working relationship between hospice staff and
nursing/medical staff in Cornwall. They will do this to ensure families have the best possible information in regard to
accessing and receiving hospice care. Major Tasks (not exhaustive): Have a CHSW caseload of children living in Cornwall
