Responsibilities

• Guide treatment plans based on patient needs.
• Establish discharge plans with the interdisciplinary team.
• Monitor patient experience and quality of care.
• Complete necessary documentation and case management addendums.
• Educate patients and families on rehabilitation processes.

Requirements

• Active RN license with a minimum of an Associate Degree in Nursing.
• CCM® or ACM™ certification required within two years.
• Two years of rehabilitation experience preferred.
• Ability to independently complete assessments.
