Clinical In-reach Assistant Practitioner
About the Role
Job Purpose To work autonomously and alongside Nurses, Physiotherapists, Occupational therapists and Assistant
Practitioners as a multi-disciplinary team member to undertake comprehensive assessment and intervention to clients
within the pre-admission areas of Nottingham University Hospitals Queens Medical Centre Campus in order to facilitate
the clients discharge or appropriate pathway identification. Dimensions To include, but not limited to, supporting the
team, providing assessments of needs, and working across professional boundaries and within legislation, policies and
procedures and core values. You will be working in a variety of pre-admission areas supporting discharge planning for
