About the Role
Job Purpose To work as an autonomous practitioner, you will undertake comprehensive assessments to identify the
appropriate service for the clients referred. To carry out profession specific assessment and evaluation as required. To
work within the MDT to plan and develop discharge plans for adults with a wide variety of complex physical and mental
health needs To be responsible for continuing the on-going development of high standards of clinical practice with
evidence-based focus within the service. To work, as required within the integrated care and Front Door Integrated
Discharge services, reacting to emergency health and social care referrals with the aim of preventing admission to
