About the Role
JOB DESCRIPTION
To provide and lead on the delivery of evidence based nursing care to children in the community by providing clinical
assessment, care planning, care delivery, care evaluation and clinical advice in accordance with the NMC Code of
Professional Conduct. Actively engage complex information in multidisciplinary activities such as discharge planning and
case conferences and maintain liaison with clients, carers and members of the multidisciplinary team. Lead in the
