About the Role

The Integrated Community Response Service is a large multidisciplinary team made up of GPs, occupational therapists, a

social worker, physiotherapists, nurses, a community matron and health and social care assistants. The main aim of the

service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge

of patients following an acute admission. This award winning rapid response team provides immediate assessment,

intervention, early rehabilitation and management for a broad range of patients with complex needs. All members of the

team work very closely together and have a good relationship with other community teams and clinicians such as district
