About the Role
The Navigator will work autonomously, possess excellent communication skills and have experience and knowledge of
working with Crisis/Rapid Response and community services within primary or secondary care services.
The post holder will be expected to undertake the holistic assessment of patients using a person-centered approach, and
no-ordinate care packages across both health and social care as appropriate to avoid inappropriate admission to hospital
or long-term care environments, in addition to facilitate the timely discharge of patients from secondary care. The
post-holder will provide holistic assessments, diagnose and formulate individual treatment plans and programmes to meet
