About the Role

An exciting opportunity has arisen for an experienced Palliative Care CNS to work within the community setting covering

a 7 day service 08:00 – 20:00 .

We are looking for an experienced, autonomous, and innovative nurse to join the integrated specialist palliative care

nursing team.

This is a Band 7 community-based palliative care CNS post, working alongside, Community Nursing Services, Consultants,

Therapists, and wider MDT to ensure that the service and team are providing expert high quality patient focused care to
