Clinical Nurse Specialist/Paramedic

About the Role

advanced practice level to provide a co-ordinated and comprehensive specialist palliative care service to patients and

their loved ones facing a terminal illness within the hospice catchment area. Working within a skill mixed team, be

responsible for a caseload of community palliative patients. This will include supporting appropriate identification and

referral of patients, conducting comprehensive holistic assessments, diagnosis, recommendation of evidence-based symptom

management plans and enable advance care planning within the community and out-patient setting.
